Step 1: Download Order Form

You may need adobe reader to download the form. We have allowed you to fill the form out after downloading without using paper. The form can not be saved for security reasons so you must fill it out and print it.

Step 2: View Catalog

After downloading the order form and getting familiar with the information you will need to fill it out, go to the catalog and search for your pictures by horse name, owner name, or rider name. Remember to write down the “Proof Id” of any images you like on the form.

Step 3: Fill out Form and Send

For you convenience, we offer you three ways to send the form after you are done filling it out. You can either fax it, order by phone, or email it back to Midge Ames. The contact information for any of these methods is listed on the form itself.
